The UTEP Miner Gold card Office offers custom cards specific to your program and its needs.  There is $25 set up fee.  Cost per card (new and replacement) are as follows:

Cost per Card	Includes
$3.00	Front print only (no magnetic stripe, no laminate)
$5.00	Front and back print only (no magnetic stripe, no laminate)
$5.50	Front and back print with magnetic stripe (no laminate)
$6.00	Front and back print with magnetic stripe and laminate

Please include the following on the Purchase Requisition Form:

• Department Name
• Contact Name
• Contact Extension
• Contact email address
• Alternative number
• Program Name
• Number of Cards Requested
• Program Start and Stop Dates
• Will the program participants need Miner Bucks accounts established for food, bookstore or copy shop purchases?  ____ yes          ___ no

If applicable, please attach and email an Excel spreadsheet with the following fields for those you are authorizing to receive your program card:

Last Name

First Name

If your program participants need University 800 Numbers, please use the following form to request the number and then complete your spreadsheet.

If a special logo, trademark, picture will be part of the card design, please email it in .jpg format to lacontreras@utep.edu .

After your request is processed, a proof of the card design will be available to view, approve or revise.

Lanyards or standard clips are available for an additional fee.
